About the Role
Responsibilities
Essential Duties:
- Serve as the primary contact for incoming inquiries, responding to calls from prospective participants and conducting timely follow-ups to support enrollment.
- Clearly communicate the Seen Health PACE enrollment process, ensuring a high conversion rate from inquiry to in-home visit.
- In collaboration with the Eligibility Specialist and Intake Specialist, address financial eligibility concerns, including Medi-Cal questions, Share of Cost (SOC) issues, and related documentation.
- With adequate training and support, perform Medi-Cal information review, application, and follow-up with DPSS until the Medi-Cal case is fully resolved.
- Demonstrate a comprehensive understanding of the MOE Playbook and apply its principles to daily operations.
- Independently schedule and facilitate Enrollment Conferences, ensuring new participants are guided through all required steps and supported throughout the initial assessment process.
- Manage scheduling for participant tours, in-home assessments, Level of Care (LOC) visits, SW and PCP assessments, Enrollment Conferences, and associated transportation.
- Support the Growth Lead in facilitating MOE team meetings, including tracking
assignments, documenting follow-ups, and logging action items in Orbit.
Outreach & Event Responsibilities:
- Represent Seen Health at community events coordinated by outreach specialists, including health fairs, mixers, and provider meetings.
- Assist in the setup, organization, and staffing of events to ensure a positive and engaging experience for attendees.
- Collect participant information during events and enter accurate data into the Orbit system, assigning leads to the appropriate Outreach Specialists.
- Educate seniors and referral partners about the PACE program and its benefits during community outreach efforts.
- Monitor inventory and prepare marketing materials and giveaways for events, ordering new supplies as necessary.
- Maintain a professional and approachable demeanor when interacting with the public at outreach events.
General Responsibilities:
- Ensure timely and accurate completion of all required documentation.
- Maintain confidentiality of participant and company information in accordance with HIPAA and company policies.
- Actively participate in team meetings, quality improvement activities, and staff training. Keep supervisors informed of work.
- Adapt to changing business needs and take on additional responsibilities as assigned.
Qualifications
- Education: Associate’s degree in a related field required; Bachelor’s degree preferred.
- Experience: Minimum of 1 years of experience in a similar role, preferably in healthcare,
outreach, or customer service. - Customer Service Skills: Strong interpersonal and communication skills with a
customer-first mindset. - Independence: Ability to manage tasks effectively with minimal supervision and adapt to
shifting priorities. - Transportation: Must have reliable transportation to attend events and conduct outreach in
the community. - Technical Proficiency: Comfortable using Mac OS and Google Workspace (Docs, Sheets,
etc.); open to learning and using new software platforms. - Language Skills: Bilingual proficiency in Mandarin or Cantonese is strongly preferred.
Medical Clearance:
Required that Team Members must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Additional screening may be required in order to protect participants and the community from communicable disease outbreaks (e.g., COVID-19).
Regulatory Requirements:
Must not be on any exclusion or debarment from participation in Federal Health Care Programs at any time and must remain in good standing with government regulators such as the OIG, (Office of the Inspector General), SAM, (System for Award Management), CMS (Centers for Medicare and Medicaid Services), etc.
Location
- Regular travel to different settings in the San Gabriel Valley community.
- In center at Seen Health in Alhambra, CA.
Salary & Benefits
- Salary: $20-$22/hr, plus commission opportunity structure
