About the Role
The Enrollment RN is responsible for conducting initial nursing and home safety assessments of prospective participants at their place of residence and managing State level-of-care (LOC) review process with Department of Health Care Services nurse. The Enrollment RN is a member of the Seen Health Marketing, Outreach, and Enrollment Team (MOE) and works in close collaboration with the PACE Physician, Social Worker, MOE team members and prospects’ caregivers to thoroughly and promptly evaluate each prospective participant interested PACE enrollment. Collaborates effectively with colleagues and stakeholders to promote the Seen Health values, team culture and mission.
Responsibilities
- Conduct initial nursing assessments, including but not limited to nursing evaluation, functional assessment and home safety evaluation, in prospective Seen Health participants’ homes, to determine if they meet PACE eligibility requirements.
- Promptly prepare documentation from home visits, submit to CA State Level-of-Care (LOC) nurse for review and respond to questions to expedite review process for participants.
- Track and manage the annual State level-of-care review process.
- Collaborate with the Seen Health IDT and MOE Team members.
- Cross-train on other MOE Team roles and provide back coverage as directed.
- Provide backup coverage to other Seen Health nursing functions.
- Comply with MOE Team’s compliance with all rules pertaining to allowable and prohibited marketing practices.
- Be legally authorized to practice in position location, required.
- Only act within the scope of his or her authority to practice.
- Complete all required documentation in a timely and accurate manner, including in CRM software.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Follow Seen Health policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
- Communicate regularly with Supervisor and team regarding workload and priorities.
- Timely completion of all mandated trainings and education.
- Timely completion of all mandated occupational health screenings as needed.
- Exercises flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
Qualifications
Education/Training/Certifications:
- Graduate of an Accredited School of Nursing
- Unencumbered Registered Nurse (RN) License with which you practice required.
- (BSN) preferred.
- DHCS Training and passing grade Exam/Certification as a Marketing Representative within 30 days of hire.
- Valid State Driver’s License with a clean DMV record.
- Copy of recent Vehicle Insurance required.
- Proficiency in Mandarin and/or Cantonese preferred.
Skills and Experience:
- Minimum of two years’ nursing experience in a clinical setting with a frail or elderly
population. - Highly motivated, self-directed, able to execute tasks in a quickly changing environment
and make sound decisions in emergency situations. - Excellent clinical, organizational and communication skills in settings with seniors, their
families and interdisciplinary team members. - Excellent organizational and communication skills.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.
Location
- Los Angeles required. Ability to commute to Alhambra required.
