About the Role

The Registered Dietitian (RD) is responsible for designing and implementing the nutrition services program working in collaboration with the interdisciplinary team (IDT). The RD conducts individual nutrition assessments, develops personalized nutrition care plans and provides dietary counseling and education to participants and their families.

Responsibilities

  • Promotes optimal health outcomes while adhering to Seen Health policies and procedures, quality improvement and regulatory compliance guidelines.

  • Conducts comprehensive nutritional assessments to determine participant dietary needs, preferences, and goals.

  • Develops and implements individualized nutrition care plans based on assessment findings, medical data, and participant preferences.

  • Collaborate with the interdisciplinary team (IDT) to integrate nutrition care into overall participant care plans and ensure coordinated care delivery.

  • Provide nutrition counseling and education to participants, families, and caregivers on topics such as meal planning, portion control, and dietary modifications.

  • Deliver and document direct care nutrition interventions in accordance with primary care provider orders (e.g. provision of nutritional supplements) and/or as agreed upon in the participants’ care plans, including but not limited to implementing and monitoring special diets, and providing nutrition counseling to participants and families.

  • Monitor participants’ dietary intake, weight changes, and nutritional status regularly, making adjustments to care plans as needed.

  • Maintains accurate and complete documentation of nutrition assessments, interventions, progress notes, and all communication with participants, healthcare providers and/ or participant caregivers in the participant’s electronic health record.

  • Oversees menu planning and meal delivery programs, ensuring cultural sensitivity (e.g. posting menus in multiple languages and offering various meal options to satisfy participant preferences).

  • Supervises compliance with safety and regulatory requirements e.g. prepares and updates nutrition meal cards ensuring that participants receive meals that meet their specific dietary needs and restrictions.

  • Direct and mentor center staff, including training and overseeing their performance in delivering nutrition services e.g. ensures that all servers are SafeServ certified and demonstrate utilization of nutrition meal cards.

  • Guide the intake and enrollment process to ensure prospective participants receive timely and effective nutrition services.

  • Collaborates with nutrition services staff and Center Director to conduct monthly and quarterly sanitation, safety and foodservice audits.

  • Participate in state and federal audit preparation activities.

  • Assist with planning and coordination of Dietetic Interns from affiliated RD and Dietetic Technician programs.

Qualifications

  • Baccalaureate or advanced degree from an accredited college with major studies in food and nutrition or related field. Must be a Registered Dietitian (RD) as confirmed by the Commission on Dietetic Registration.

  • Minimum of 1 year’s experience working with a frail or elderly population.

  • Three (3) years of experience working as a Registered Dietician in geriatric or managed care setting.

  • Excellent leadership, organization, and communication skills in settings with seniors, their families, and interdisciplinary team members.

  • Ability to translate best practices into culturally appropriate resources and work effectively with diverse populations.

  • Bilingual English/Mandarin/Cantonese preferred.

  • Current Basic Life Support (BLS) / First Aid Certification required
