ABOUT THE ROLE
We are seeking a talented and skilled Population Health Associate to work directly with our Central and Market teams to build, teach, implement, and observe workflows for our population health operating model. The role will support core population health domains including but not limited to workflow optimizations and data gathering that help drive excellence in clinical documentation, utilization, and clinical quality. This role will report to the Population Health Director.
During the Transformation and Launch phases of our Care Model Implementation, the Population Health Associate will work in the clinics, supporting the Clinical Information. This role will require travel and will involve work across communities in Western NC for weeks at a time.
Specific responsibilities include (but are not limited to):
During Implementation/Transformation/Launch (in clinic work):
- Supporting Providers and CISs through their initial course work, ensuring completion and taking feedback on any areas of confusion or necessary follow-up
- Providing role-based training to CISs including use of Athenahealth
- Working side by side with the CIS during onboarding/training to ensure proper use of chart prep workflows, daily huddles, suspecting, and scribing
- Using checklists to document the effectiveness of each CIS and escalating areas that need further support
- Observing and assisting in the CIS/Provider relationship building, ensuring they are communicating and collaborating in the most effective way to yield accurate documentation and quality patient care
- Assisting the Population Health Manager in building and delivering new content and programs related to Clinical Documentation
- As assigned, completing chart audits for the purposes of ongoing education on accurate documentation
When not in market supporting the above:
- Developing, testing, and troubleshooting all field facing tools and data related to Clinical Documentation, Quality, and Utilization.
- Partnering with IT/Foundry developers on development, design, functionality, and reporting needs as it relates to Clinical Documentation, Quality, and Utilization tools
- Identifying opportunities for improvement in logic or tool functionality and illicit input from various cohorts including but not limited to PHD, CISs, CMO/Medical Directors, Providers.
- Ensuring all tools are functioning as intended and partnering with Finance for quick resolution if troubleshooting is necessary
- Other duties as assigned
ABOUT YOU
We are looking for a motivated, mission driven, and experienced leader with:
- 1+ years' experience at a consulting firm or similar analytics experience
- Prior experience in healthcare, population health, working with providers, or value-based care models a plus
- Bachelor’s Degree required
- Ability to analyze data to build out a business case or ROI analysis
- Experience working with multiple software & data platforms, electronic health records, spreadsheet development and navigation, data processing, organizational skills and ability to manage multiple priorities appropriately
- US work authorization
