About the Role
As the Physician Site Leader for our [Location] clinic, you will deliver high-quality care, an exceptional patient experience, and a best-in-class culture, while providing day-to-day leadership for the clinic team and acting as a Champion of the Hopscotch Care Model and a Culture advocate for your team and your patients.
Hopscotch Care Teams are responsible for delivering high-touch, high-quality care to patients, including coordination of care across different physicians and the broader healthcare continuum.
What You’ll Do
In this role, you will:
- Lead, oversee and support the team to drive day-to-day execution against critical priorities, a great patient experience, team engagement and a best-in-class culture
- Define objective goals and top priorities for the clinic and team members
- Define team and culture priorities
- Partner with Care Center Manager to manage operational needs proactively and collaboratively
- Champion the Hopscotch Care Model and brand in the community
- Set high quality standards for the Care team and across Care Centers to enable the model
- Lead by example and drive execution across team for all critical elements of clinical workflows (e.g. documentation and coding)
- Lead Daily Huddles, High-Risk Huddles, Clinical Forums, trainings, and process rollouts to drive patient and overall clinic outcomes
- Ensure all Providers complete administrative tasks that support care model
- Provide high-quality patient care in the comprehensive and interdisciplinary care model of Hopscotch Health to deliver best-in-class outcomes and results for your dedicated patient panel
- Provide direct oversight of your clinical care team (APP, MA, CHW, etc.)
- Deliver comprehensive primary care services to a dedicated panel of patients
- Develop trust with patients and caregivers
- Drive excellence with a “continuous improvement” mindset
- Outperform target metrics and outcomes in collaboration with your care team
- Proactively communicate across the care team and advocate for the patient
- Support the Hopscotch mission and vision through your leadership around talent and culture, clinical innovation and compliance/best practices to enable success
- Act within the Hopscotch “Code of Conduct” at all times and live the Hopscotch mission and values
- Demonstrate commitment to value-based care and compliance in clinical leadership and ensuring standards are well-defined and met
- Represent Hopscotch Health in the community, participating in events and providing community-based education and engagement
- Foster a culture of communication, collaboration, and positivity with a focus on results and accountability
- Help attract, develop, and retain talent across the organization
- Monitor and understand best practices and evidence-based approaches in care and work to incorporate across Hopscotch
- Message concerns and risks regarding patient care, team, or culture to leadership and propose solutions

About You
You’ll be a great fit for this position if you have 5+ years of post-Residency experience and have experience in managing teams. You must be Board Certified in Internal Medicine or Family Medicine or Geriatrics. Our ideal candidate will have experience with and/or robust knowledge of:
- Value-based care and other innovative clinical models that serve high acuity members
- Experience with Medicare population
- Caring for “at-risk” panel of patients
- Using data and insights to drive action in a clinical setting
- Leading, teaching, and training others, in formal and informal ways
Additional Qualifications include:
- Active, non-probationary North Carolina state medical license
- U.S. work authorization
