About the Role

The Integrated Care Design manager is responsible for connecting Cityblock’s care model with new external partners to prepare to serve new members. After partnership discussions have advanced to a stage where Cityblock plans to open a new line of business, the manager will be the subject matter expert to external partners for Cityblock’s care model offerings. They will lead external sessions to help partners understand Cityblock’s care model and configure clinical workflows to meet unique partner needs while maintaining the core Cityblock model and ensuring our members receive the best possible care. The manager will collaborate closely with internal stakeholders across the organization to ensure preparedness for new implementations and to continue to support Cityblock’s clinical growth strategy.

Responsibilities:

  • Lead sessions with external partners (ex. payer and health system directors) explaining offerings within Cityblock’s Care Model and provide a clinical perspective to external clinical stakeholders

  • Lead configuration sessions with external partners connecting Cityblock workflows with partner operations, finding balanced solutions that meet Cityblock and partner needs while ensuring members receive the best possible care (Lucid, Slideshow)

  • Develop and organize clinical partnership and care model assets for internal and external use to ensure consistent processes during new partner implementation

  • Collaborate with internal stakeholders to communicate new partner needs for effective member care, ensure preparedness for new implementations, and facilitate partnership workflow decisions that advance Cityblock’s mission and business

  • Collaborate with internal stakeholders to continue to develop Cityblock’s clinical strategy and growth

Requirements:

  • Bachelors Degree

  • 5+ years of professional experience in the healthcare industry, with a focus on care model design, program management, clinical ops support, and/or health plan partner collaboration

  • Clinical training and certification preferred, not required (LCSW, RN, PA, NP)

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:

$115,000 - $150,000
