About the Role:
The Cityblock (CBH) Associate Market Medical Director is responsible for providing medical oversight to clinical staff across our home and community-based care teams and multiple Cityblock clinic locations in the market, focused on our different lines of business.. You will collaborate with our partners, and lead the development and refinement of clinical practices and policies.
In partnership with the VP, [Market] Medical Practice, the AMMD collaborates with our health plan partners, and leads the development and refinement of clinical practices, strategic operations, and policies. The role works collaboratively with an interdisciplinary team of clinical and non-clinical leadership to support the market to hit their gross margin goal, clinical quality goals, and other essential market-specific goals.
This role leads with passion for high quality, value-based care, operates and manages a large distributed workforce in a matrixed environment, and leads by example, with empathy, and positive influence to support the continuous improvements of our clinical care services.
Strategy
Develop and implement cost of care initiatives based on segmentation and other population health drivers and partner with your local care teams to further evaluate and iterate initiatives to support high quality member care, for both consented and pre-consented members
Partner with Cityblock’s Data and Analytics teams to understand and map solutions to address drivers of utilization and translate this information to local care teams for iteration of potential data-driven solutions
Lead clinical quality improvement initiatives that may span multiple locations and that focus on HEDIS, contracted or other determined clinical quality metrics, in partnership with Cityblock’s Quality Improvement team
Decrease unnecessary hospitalizations, emergency room visits, and unnecessary speciality referrals
Develop and implement strategies to right set Medicaid and Medicare revenue with the VP, Medical Practice and central BoI teams.
Clinical Care and Oversight
Collaborate with Clinical Staff Affairs to manage provider licensing, credentialing and ongoing competency requirements in order to assure market providers are adequately privileged for clinical care
Manage market patient safety efforts and clinical risk management, achieving practice-level and individual patient quality outcomes, in partnership with Cityblock’s Patient Safety team
Participate in the delivery of educational content around complex care delivery including geriatrics, palliative care, chronic disease management, social determinants of care, and integration of primary and behavioral health needs, across the entire care team including non-clinical staff
Provide clinical care to an appropriate panel of members with complex needs, with an emphasis on chronic disease management, primary behavioral health, primary palliative care and substance use disorder diagnosis and treatment
Work a full-time week and be prepared to join the Cityblock on-call schedule as needed. You may be expected to participate in a Saturday Rotation based on your market’s schedule. You may need to work outside of regular business hours to execute on the market’s clinical or business needs.
Meet evolving monthly and quarterly targets for visits with Cityblock members, in both clinic and home setting
"Direct report providers report feeling supported by you, their manager: Minimal voluntary attrition
Clinical cases of Providers meet standards for evidence based complex primary care
Attend case conferences on regular basis to advise on clinical needs for presented members and establish goals as part of members' care plans.
Partner and network management
Attend external meetings and activities to further the business goals of the organization
Operations
Operate in lockstep with the Hub Operations team, to ensure the clinical needs are met by practice management, flagging any concerns or processes that you experience or that are raised by your team.
Requirements for the Role:
Graduate from an accredited medical college (within the United States/Canada) or from an unaccredited medical college (outside of the United State/Canada).
Board certification in a major primary care specialty field of practice. Certified in Basic Life Support for Healthcare Providers DEA licensure (or willingness to acquire)
MD, DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required
Current valid license to practice medicine in the state in which you are employed with Cityblock.
Willingness to acquire licenses in other states according to CBH business need.
3-5 years clinical team leadership
Are comfortable using technology to support members (telephone and text etiquette, virtual visit platforms, scheduling platforms etc.) and to communicate with interdisciplinary team (slack, google documents, etc).
Consolidate and understand feedback from end users of the electronic health record systems and/or care facilitation platforms and communicate to cross-functional teams to troubleshoot and optimize systems.
Document thorough medical, behavioral and social history with clearly communicated assessments and plans.
Expert in Motivational Interviewing and Trauma informed Care principles.
Able to model and teach these principles to the team.
Have strong problem solving skills - can make difficult decisions and knows when and how to collaborate with cross-functional leadership and other cross-functional team members and leadership.
Able to provide creative solutions to challenges within the healthcare system that are impeding members’ care and health.
Analytical and empirical -- focused on optimizing return on investment of resources against stated objectives.
Knowledgeable about what drives outcomes in this space, including strategically driving population health programs that reduce TCOC and increases capitation revenue (Medicare & Medicaid revenue optimization).
Have experience building and implementing new or evolving programs and processes.
Understands company Objective and Key Results and contributes to program strategy through data-driven approaches.
Play a lead role in monitoring, evaluating, and driving quality improvement activities across hubs, home-based care, and virtual care. This role includes review of member and care team quality of care concerns.
Drive integration of clinical innovations, including community medicine, care transitions, palliative care, and others, in line with enterprise strategy and programs with an eye towards deploying measure-able programs that reduce TCOC and right-sets capitation revenue.
Oversee and be accountable for the performance of in-market providers and other clinical performance.
Strategically build, deploy, and measure programs that decrease IP/ED utilization, decrease other areas of impactable spend (ex. LTSS), and right-sets capitated Medicaid and Medicare revenue.
Able to communicate to team about their performance based on metrics that drive intended outcomes.
Understands how to motivate and drive teams and has experience building and managing clinical teams.
Ability and experience supervising Physicians and APCs (3-5 year minimum)
